We are NOT a health insurance plan, a Medicare Insurance Supplement, a Medicare insurance supplement, a health discount plan, or a prescription discount plan.
Use of Membership: Your Membership is non-assignable, non-transferable, and cannot be resold.
Membership Term: Your Membership is effective immediately upon Your subscription payment and enrollment in the plan and shall continue on a month-to-month basis. Your benefits will be calculated on a yearly basis(twelve month period), starting the day of subscription (‘Membership Year”).
We operate subject to state regulations and prescriptions may not be available in Your state. As of now, we provide services in NY, NJ, NC, FL and MI. We are expanding our services to further states.
Membership Plans and Benefits: As a Member, You are entitled to access our services offered at the following terms, subject to rates as provided on our website and App and might change from time to time according to the sole decision of Antidote, in respect to each and every Membership Plan:
Includes: 12 Doctor visits for acute and primary care needs and telehealth permitted prescription renewals during the Membership Year.
Beginning with the 13th visit, each subsequent visit during that Membership Year is subject to a copay of $15.
Additional benefit: Acute and Primary Care Drugs prescribed by our doctors during Your Membership period (up to $100 per Membership Year).
Unlimited family members (spouse, grandparent, children)
Including an option for children under the age of 18 - Child mobile App that enables the child to participate on a call with a doctor.
Personal Family Doctor
Includes: 20 Doctor visits for acute and primary care needs and telehealth permitted prescription renewals, for whole family, during the Membership Year.
Beginning with the 21st visit, each subsequent visit during that Membership Year is subject to a copay of $15.
Additional benefit: Acute and Primary Care Drugs prescribed by our doctors during Your Membership period (up to $150 per Membership Year).
Personal Family Doctor: If You are a member of our Family Plans (Family Membership, Family Membership+), You will be able to choose a personal family doctor to treat Your family, from our list of experienced and professional physicians offered. Choosing a designated doctor will not prohibit You from choosing to see any other physician when using our service.
Drugs included in the Membership+:
Our Membership+ Plans (Individual Membership+ and Family Membership+) include Acute and Primary Care Drugs, up to the limit as set in each plan.
In case that our physician prescribes You with Primary Care Drugs that are included on the list of Membership Included Drugs, the Antidote system will create and activate a virtual Debit Card for you with funds of up to $15 per prescription. The funds are assigned to your personal Antidote Debit Card will expire after 10 days. You can use the Antidote Debit Card for the prescription transactions only.

Payment of Membership Fee:
Upon enrollment, Antidote will charge You for Your first month of Membership. Your date of enrollment will begin your first Membership Month. You will then be charged monthly in advance on an ongoing basis, for Your Membership, on the same day, or in close proximity to that day, each subsequent month.
You authorize us to charge Your payment source, the applicable monthly membership fee, according to Your chosen Membership Plan, and rates provided on our website and App. Prices do not include applicable taxes, which are determined according to Your billing address.
You may also choose to pay annually with a discount as offered from time to time on our website and App.
Cancellation and Refunds
If you wish to CANCEL Your Membership You may do so at any time. The benefits of the Antidote Membership may be enhanced or modified at any time without prior notice. If You are not completely satisfied, simply email us at email@example.com to cancel Your Membership. The Cancellation will go into effect the Membership Month following Your cancellation notice, and no further charges will be made to Your provided payment method, and there will be no further obligation.
If You cancel Your Membership, you will be able to continue using our services as a one-time visitor, and You can re-enroll into one of our Membership Plans, after a 6 month waiting period.
Except to the extent that applicable law explicitly requires a longer notice period, we reserve the right to terminate Your Membership at any time without notification, including in the event that we are unable to bill the membership to Your payment source.
During any Membership Month in which You have not used our services, You have the right to cancel Your monthly payment (i.e. - actual payment, if credited for drugs) within the first seven (7) days of that Membership Month, and receive a refund for that month's Membership Fee payment. In the event that applicable law or regulation requires a longer refund period, we will honor such a longer period. To request a refund please contact us via email at firstname.lastname@example.org.
- For NY residents: if you are a NY resident, you will be able to cancel your Yearly Membership at any time as mentioned above, if you had, on average, one (or less) Doctor Visit and/or prescription refund, each Membership Month. If you exceed an average of one Visit and/or prescription refund each month, you will have the right to cancel Your Membership only after 6 months from your first day of subscription.
My Antidote Debit Card
As part of Your Membership Plans Plus (Individual Plus and Family Plus) you will be issued a My Antidote Debit Card (a STRIPE issued Spend Card). The Card will be issued to you to able Us to pay for Medicine included in the plan, or any other Medical services and benefits.
You are committed to use the card only for these purposes and not abuse the card for other purchases. Any abuse of the card will cause us to cancel the card, your benefits and Membership.
The Stripe issued Spend Card is a business charge card issued under the Stripe Issuing: Spend Card Program (the “Program”). The Stripe Issuing Spend Card is issued by Celtic Bank, a Utah-chartered, Federal Deposit Insurance corporation-insured bank located in Salt Lake City, Utah (“Issuer”), and the Program is managed by Stripe, Inc., a Delaware corporation (“Stripe”). These Authorized User Terms (“Terms”) apply to your use of the Stripe Issuing Spend Card. Capitalized terms used in these Terms, but not defined elsewhere, have the meaning assigned to such terms in Section 1.
As used in these Terms, “we”, “us” or “our” refers to Stripe, Issuer and Antidote. “You” and “Your” refers to the person or persons authorized by Antidote that participates in the Program (the “Program Accountholder”) to receive and use a Stripe Issuing Spend Card.
You agree that Stripe may provide you with electronic communications.
(a) “Administrator” means the person appointed by Antidote to manage the Program Accountholder’s participation in the Program, including managing how you may use your Stripe Issuing Spend Card.
(b) “Card” and “Stripe Issuing Spend Card” mean a business charge card issued by Issuer, linked to a Card Account, and enabled for Transactions over the Card Network associated with the Card. Cards may either be a physical card embossed with a 16-digit number or a card represented by a 16-digit number.
(c) “Card Account” means a sub-account of the Program Account that is linked to a Card issued to you.
(d) “Card Limits” means the maximum amounts available for Charges on any Card. Your Administrator will inform you of Card Limits that apply to your Card.
(e) “Card Network” means the payments card network operated by Visa U.S.A., Inc. and/or Visa International and/or MasterCard International Incorporated.
(f) “Charge” means the use of your Card to complete a Transaction.
(g) “Data” means personal or business information provided to Stripe in connection with the Program, which includes information to determine a Program Accountholder’s eligibility for a Program Account and other information needed by Stripe to provide the Program and to fulfill its responsibilities to Issuer.
(h) “Personal Data” means information that identifies a specific living person (not a company, legal entity, or machine) and is transmitted to or accessible through the Program.
(i) “Program” and “Stripe Issuing Spend Card Program” mean the program through which Issuer issues Cards and extends credit to Antidote, and Stripe provides Card Accounts, Cards, and Transaction management tools and services, in each case for use by the Administrator and you.
(j) “Program Account” means the account for which Antidote may request the issuance of Cards, and includes the records Stripe maintain to (a) account for the value of the funds available for Charges on Cards, (b) establish Card Accounts, (c) provide Transaction histories (Charges, Transactions, payments), and (d) inform Antidote of fees and payments it owes.
(h) “Transaction” means a request initiated by you to make a payment with a Card for the purchase of goods or services.
2. My Antidote Debit Card and Card Account.
Your Card is issued and provided by Issuer and is linked to an individual Card Account. Your individual Card Account is linked to Antidote’s Program Account. Your Charges are billed to the Program Account. You are not directly liable to Stripe for your Charges or other activity on your individual Card Account. Your Card must be activated prior to use. Antidote’s Administrator will provide activation instructions to you. If you did not receive activation instructions for your Card, contact your Administrator. We may apply a Card Limit to your Card Account. Charges will reduce the Card Limit, and any Charges in excess of the Card Limit may be declined.
3. Account Opening.
IMPORTANT INFORMATION ABOUT PROCEDURES FOR OPENING A NEW ACCOUNT: To help the government fight the funding of terrorism and money laundering activities, federal law requires us to obtain, verify, and record information that identifies you when a Card Account is opened for you. What this means for you: In connection with opening your individual Card Account, we will ask you for your name, street address, date of birth, and other information that will allow Stripe to identify you. We also may ask to see your driver’s license or other identifying documents from you.
4. Using Your Stripe Issued My Antidote Debit Card.
You may only use My Antidote Debit Card for bona fide business purchases on behalf of Antidote and for valid and lawful purposes. You may not use your Card for personal, family or household purposes. Your Card remains the property of the Issuer and must be returned upon request. We may cancel, revoke, repossess or restrict the use of your Card at any time. We may decline to authorize or reverse Charges or suspend your Card for any reason including violation of these Terms or suspected fraud. We also may decline to authorize Charges at merchants characterized by the Card Network or us as prohibited merchants. Stripe are not responsible for losses resulting from declined or reversed Charges. Merchants typically accept all Card Network-branded Cards; however, Stripe or Issuer are not responsible and will have no liability if a merchant refuses to honor a Card or accept a Transaction.
5. Card Account Holds.
We may place a hold on available funds in your Card Account for certain Transactions. When a hold is placed on your Card Account, the amount of the hold will reduce the then-applicable Card Limits until the hold is released.
6. Card Security.
You are responsible for securing your Cards, account numbers and Card security features (including the CVV and PIN).

You must engage in the Program in a lawful manner, and must obey all laws, rules and regulations that apply to your use of the Program
9. Prohibited Persons.
You represent and agree you are not currently and will not become subject to a U.S. Office of Foreign Asset Control list, or any law, regulation, or other list of any government agency that prohibits or limits us from providing Card Accounts or Cards to you or from otherwise conducting business with you.
